BaZi Natal Chart Intake Form

We only collect the information required to prepare your report. No personal name is required for analysis.

Existing clients: enter your Gé Rén ID — the number assigned to you (e.g., 260719881). You may include your nickname or initials if you wish.

Used only for communication and delivery of your reading. A dedicated email is recommended for privacy.

Accurate birth information is essential. A 1–2 hour difference may change the analysis. If unsure, please check hospital records or confirm with family first. Please do not guess — if your birth time still cannot be confirmed, let us know.

Gender is used for accurate BaZi analysis. If you prefer not to disclose it, we can prepare both versions for comparison. This requires double analysis and therefore an additional fee.

Focus Areas (4 included)

Select at least four Focus Areas you would like Gé Rén to include in your BaZi Natal Chart report. Four standard Focus Areas are included with your report. If you select more than four, or if you select “Other,” each additional item is charged at $60 CAD. Gé Rén will confirm your final total on the invoice — no payment is taken through this form.

This form is for one BaZi Natal Chart report. Any add-ons will be confirmed on your invoice.

Payment Note — Gé Rén does not require your real name for BaZi analysis. Any name requested during invoice payment is part of standard billing validation. Gé Rén will use your intake form details, email address, order record, and assigned Gé Rén ID for service communication and report preparation.

Privacy Statement

Gé Rén is a privacy‑first service.

We only collect the information required to prepare your report: birth date, birth time, birth location, current location, and email.

We do not require your legal name for analysis.

If you choose to pay by credit card, your name is collected only by the payment processor for verification and receipt purposes.

Gé Rén does not store your name in your intake record and does not link it to your birth data.

Your information is used only to prepare your personalized report and support related email follow-up.
